Definition

Bei der Organisation eines thrombosierten Gefäßes eintretende, primär endovasale, seltener extravaskuläre (z.B. bei Organisation eines Hämatoms), stadienhaft ablaufende, reaktive (gutartige) Endothelproliferation. Eine Ausdehnung auf das perivasale Bindegewebe ist in späteren Phasen möglich. Dieser Proliferationsmechanismus kann auch in präexistenten vaskulären Tumoren oder Malformationen, z.B. in Hämangiomen, Granuloma pyogenicum oder Naevus flammeus auftreten.

Lokalisation

Meist Haut oder Subkutis von Extremitäten (Finger), Kopf, Nacken oder Schulter; auch im Bereich der Mundschleimhaut (Vieira CC et al. 2021); selten im Genitalbereich.

Klinisches Bild

Meist an ein größeres Gefäß assoziierter, blauroter, tief dermal oder subkutan gelegener, fester, sich langsam vergrößernder Knoten von unterschiedlichem Ausmaß (Ø max. 4-5 cm), der klinisch nicht unbedingt als Gefäßläsion erkennbar sein muss.

Histologie

Meist noch erkennbare, größere, venöse Gefäße mit thrombotischem Material, das von Endothel-ausgekleideten Septen und Papillen durchsetzt ist. Schwammartiger Aspekt mit bizarren, miteinander kommunizierenden Hohlräumen. Keine Endothelmitosen, kein Multilayering.

Differentialdiagnose

Vom histologischen Aspekt alle Neoplasien mit endothelialen Papillen, z.B. Angiosarkom, retiformes Hämangioentheliom, endovaskuläres papilläres Angioendotheliom.
