Definition

Degeneration der peripheren Nerven der hinteren Rückenmarkswurzeln ungeklärter Ursache mit konsekutiver Polyneuropathie und Auftreten schmerzloser, tiefer Ulzerationen an Füßen und Händen. Der sehr seltene HSN Typ II unterscheidet sich von dem HSN Typ I durch eine das frühzeitige Eintreten der klinischen Symptomatik sowie durch die deutliche Einbeziehung der Hände.

Ätiopathogenese

Autosomal-rezessiver Erbgang, geringe Penetranz, variable Expressivität.

Bei einem 1. Genotyp (OMIM 210300) liegt eine Mutation im WNK1-Gen vor, das auf Chromosom 12p13.33 lokalisiert ist und für WNK lysine deficient protein kinase 1 kodiert.

Bei einem zweiten Genotyp (OMIM 613115) liegt eine Mutation im FAM14B -Gen vor, das auf Chromosom 2q37.3 lokalisiert ist und für Reticulophagy receptor FAM134B kodiert.

Bei einem dritten Genotyp (OMIM 614213) liegt eine Mutation im KIF1A-Gen vor, das auf Chromosom 2q37.3 lokalisiert wird und das für Kinesin-like protein KIF1A kodiert.

Bei einem vierten Genotyp (OMIM 2430000) liegt eine Mutation im SCN9A-Gen vor, das auf Chromosom 2q24.3 lokalisiert wird und das für Sodium voltage-gated channel alpha subunit9 kodiert.

Manifestation

Säuglings- Kleinkindesalter, männliches Geschlecht bevorzugt.

Lokalisation

Untere Extremitäten; im Gegensatz zum HSN Typ I sind die Hände mteinbezogen.

Differentialdiagnose

Hereditäre sensible Neuropathie Typ I; Syringomyelie, Acropathia ulcero-mutilans non-familiaris, Tabes dorsalis, Lepra, Polyneuropathien (Alkohol, Diabetes mellitus, Vitamin B1-Mangel).

Komplikation(en)

Oysteomyelitis mit Gefahr einer Sepsis

Therapie

Symptomatisch: Vermeidung örtlicher mechanischer Reize und Druckstellen (z.B. orthopädische Schuhe), Anleitung zur täglichen genauen Selbstuntersuchung auf kleinste Verletzungen, Behandlung der Ulzera mit antiinfektiösen und wundheilungsfördernden Substanzen, s.u. Wundbehandlung. Zusammenarbeit mit Neurologen und Orthopäden.

Verlauf/Prognose

Langsam progredientes Fortschreiten der klinischen Symptomatik
